I just tried this with the Network Installer ISO and a local mirror on the hard drive which also works.
I also tried the all.img

The question I have is, is the installer deliberately ignoring the 4th hard drive partition which is here my local mirror is located or is it an oddity of my system?
If someone can confirm my findings I'll file a bug report.

It was a badly partitioned hard drive "overlapping partitions".
I'll mark as solved.
